The checklist can be used by assessment/case management staff who are assisting an individual with referral and placement after a level of care assessment or can be used by a treatment provider when transfer or referring an individual to the next level of care.

Please contact the Approval of Care Department if you have any barriers placing an individual for inpatient or outpatient levels of care.
